Do Not Sell or Share My Personal Information

At PhysioFitLab, we respect your privacy and are committed to protecting your personal information. In accordance with applicable state privacy laws (including the California Consumer Privacy Act, as amended by the CPRA), you have the right to opt out of the “sale” or “sharing” of your personal information (including sharing for cross-context behavioral advertising).

Submit an Opt-Out Request

Please email your request to support@physiofitlab.com with the subject line “Do Not Sell or Share My Personal Information.” Include the following details:

• Full Name

• Email Address

• Phone Number (optional)

• Address (optional)

• Reason for Request (optional)

• Additional Information (optional)
  If you are submitting as an authorized agent, also include your authorization documentation.

How We Process Your Request

• Verification: We may request additional information to verify your identity (or your authority as an agent). We will use this information solely to process and document your request.

• Effect of Opt-Out: Once verified, we will honor your request and cease “selling” or “sharing” your personal information as defined by applicable law.

• Confirmation: You will receive a confirmation email once your request has been processed.

Your Rights (CCPA/CPRA)

If you are a California resident, you may have the following rights (subject to exceptions):

• Right to Know/Access: Request the categories and specific pieces of personal information we collect, use, disclose, sell, or share.

• Right to Delete: Request deletion of personal information we collected from you.

• Right to Correct: Request correction of inaccurate personal information.

Minors

We do not knowingly sell or share the personal information of consumers under 16 years of age. For consumers aged 13–16, an opt-in is required; for consumers under 13, verifiable parental consent is required.
